Clinical Profile and Urological Complications Among Patients with Pelvic Fractures: A Prospective Cohort Study.

Rezumat

Background:

 Pelvic fractures frequently result from high-energy trauma and can coexist with injuries to the bladder, urethra, kidneys, or ureters. Delayed recognition of these injuries increases morbidity and complicates orthopaedic management.

 Objectives:

To describe the clinical profile, fracture patterns, urological complications, management, and short-term outcomes of patients with pelvic fractures and to identify factors associated with urological involvement.

 Methods:

This prospective cohort study included 100 adults with radiologically confirmed pelvic fractures treated at two tertiary teaching hospitals between October 2025 and March 2026. Demographic details, mechanism of injury, clinical findings, Tile fracture classification, associated injuries, urological involvement, treatment, complications, and outcomes were recorded. Categorical variables were analysed using Pearson’s chi-square test and Fisher’s exact test, as appropriate, and continuous variables using the independent-samples t-test.

 Results:

The mean age was 40.2 ± 15.7 years; 78% were males. Road traffic accidents accounted for 71% of injuries. Type B fractures were most frequent (46%), followed by type A (30%) and type C (24%). Urological involvement occurred in 33% of patients, including bladder injury in 14%, urethral injury in 10%, renal injury in 5%, ureteric injury in 1%, and urinary retention without structural injury in 3%. Urological involvement was significantly associated with Tile type C fractures, haemodynamic instability, open fractures, pubic symphysis diastasis, gross haematuria, and delayed presentation. Affected patients had longer hospital stays, more intensive care admissions, and higher in-hospital mortality.

 Conclusion:

Urological involvement was common and was concentrated among patients with severe or unstable pelvic injuries. Gross haematuria and pubic symphysis diastasis were important warning features.

 Recommendations:

 Early multidisciplinary assessment and selective cystographic or urethrographic evaluation should be incorporated into pelvic trauma protocols.
