Rezumat

Pulpectomy is widely regarded as a safe and effective treatment for grossly carious or non-vital primary molars, allowing preservation of the tooth and its space-maintaining function until natural exfoliation. Nevertheless, a range of deleterious sequelae have been reported following the procedure, including arrest of development of the permanent successor, deflection of the permanent tooth bud, foreign body reaction, and, less commonly, the formation of a dentigerous cyst in relation to the periapical region of the pulpectomised primary molar. Because a well-fitting stainless steel crown (SSC) can keep the tooth clinically functional and asymptomatic, an underlying inflammatory process may progress silently and present later as a sizeable lesion. This case report illustrates a nine-year-old boy who presented with a three-month history of a gradually enlarging extra-oral swelling of the left lower jaw, in whom cone-beam computed tomography (CBCT) revealed a well-defined unilocular radiolucency enveloping the crown of the erupting permanent second premolar (35), contiguous with a primary molar (74) that had previously undergone pulpectomy and stainless steel crown restoration. The lesion was managed by extraction of the primary tooth combined with surgical enucleation of the cyst under general anaesthesia, with careful preservation of the permanent successor. Histopathological examination confirmed an inflammatory dentigerous cyst. In general, the prevalence of cystic lesions associated with pulpectomy-treated primary molars is extremely low, less than 3.3% of all periapical lesions in primary teeth. This case underscores the importance of maintaining a high index of suspicion for periapical pathology in asymptomatic, crown-restored, pulp-treated primary teeth, and highlights the value of timely radiographic evaluation and surgical intervention in preserving the developing permanent dentition.
